Dry and Itchy Skin

Itch without a clear rash, scaly skin on the legs and hands, or itch that gets worse at night. Some causes are in the skin, and some are internal.

dry itchy skin

Quick answer

Itch without a clear rash most often comes from skin that has lost its moisture, and it is common in older people, after hot showers, and in those who work with cleaning agents. But itch that is widespread, persistent and does not respond to moisturiser can be a sign of an internal condition such as a thyroid, liver or kidney problem, iron deficiency or a side effect of medication. The doctor will assess the skin first, and consider blood tests if the pattern of the itch points to an internal cause.

When an itch should be examined

Itch all over the body with no rash

Widespread itch with no clear rash is a pattern that should be assessed, not simply moisturised.

No better even with moisturiser

If moisturiser has been used consistently for two to three weeks with no change, another cause should be considered.

Itch worse at night

Night-time itch that disturbs sleep should be examined, including for scabies if anyone else in the house has the same itch.

Weight loss or tiredness along with the itch

Itch with weight loss, unusual tiredness or a change in appetite should be assessed with blood tests.

Yellowing skin or dark urine

Itch with yellowing of the skin or eyes needs to be examined promptly.

Itch after starting a medication

Several medications cause itch without a rash. Bring your medication list when you come.

Causes in the skin itself

The most common cause is dry skin. When the outer layer of the skin loses water, it becomes rough, finely scaly and itchy, especially on the shins, forearms and hands.

Several things speed this up: long hot showers, soap that strips the skin oils, air-conditioned rooms all day, work that involves wet hands, and age, because the skin produces less oil as we get older.

Scabies also deserves a mention, because it is often mistaken for dry skin. It is caused by a very small mite, the itch is far worse at night, it appears between the fingers, on the wrists and around the waist, and it is contagious. If more than one person in the house is itching, this should be examined.

When an itch points to an internal cause

Some itch does not begin in the skin. In these cases the skin looks almost normal apart from the marks of scratching, and moisturiser does not help much.

Conditions that can cause widespread itch include thyroid problems, liver disease and kidney disease, iron deficiency, uncontrolled blood sugar, and several types of medication. More rarely, persistent itch can be an early sign of a more serious condition.

That is why the doctor will ask questions that seem unrelated to the skin: weight, appetite, energy, bowel habits, and the medication you take. If the pattern points to an internal cause, basic blood tests can be suggested, and they are explained before they are done.

What usually helps

For itch caused by dry skin, these have the clearest effect.

  • Bathe in warm water, and not for longer than you need to
  • A fragrance-free cleanser that does not strip the skin oils
  • Moisturiser applied within a few minutes of bathing, while the skin is still damp
  • Moisturiser repeated at night for the driest areas
  • Loose cotton clothing, avoid materials that abrade the skin
  • Nails kept short, because scratching itself adds to the itch and damages the skin

Common questions

Does itch without a rash mean it is not serious?

Not necessarily. Widespread, persistent itch without a rash is a pattern that should be assessed, because some of it comes from inside the body.

Do I need blood tests?

It depends on the pattern. If the itch is localised and the skin is clearly dry, usually not. If the itch is widespread, persistent and moisturiser does not help, the doctor may suggest basic tests and will explain why.

Why is it itchier at night?

Body temperature rises under a blanket, and there is nothing else to distract you. But very severe night-time itch, particularly between the fingers and on the wrists, should be examined for scabies.

Is scabies contagious?

Yes, through prolonged close contact and shared bedding. If it is found, other household members usually need treating at the same time even if they are not itching yet.

What kind of moisturiser is best?

One that is fragrance-free and thick enough for your skin. Greasier ones are usually more effective but less pleasant. Pick the one you are willing to use every day, because consistency matters more than the brand.

My elderly parent itches badly. Is that normal with age?

Skin does become drier with age, so itch is more common. But severe itch in an older person should not be assumed to be normal without being examined, because medication and internal conditions are more often involved in this group.

Can I use cooking oil or calamine powder?

Some people get a little relief, but it is not a treatment. Powder can dry the skin further. If the itch persists beyond two to three weeks, come and be examined.
