Quick answer

Acne is not a single condition. Comedones, inflamed pustules and deep cystic acne need different approaches, and a medication that suits one type may do nothing for another. The doctor will determine the type and severity, then explain the treatment options and a reasonable timeframe. Acne usually takes six to twelve weeks to show a clear change.

When acne should be seen by a doctor

Painful cystic acne

Deep, firm, painful lumps under the skin. This type most often leaves scars, and creams on the surface are rarely enough.

Three months with no change

If pharmacy products have been tried for three months without change, switching products again is usually not the answer.

Starting to leave scars

Scars are easier to prevent than to treat. As soon as you see a depression or a permanent mark, stop waiting.

Acne on the body and back

Acne on the back, chest and shoulders is harder to reach and more often needs oral medication than facial acne alone.

Started after a new medication

Some medications, including hormonal medication and steroids, can trigger acne. Bring a list of your medication.

Irregular periods with acne

Acne together with irregular periods or excess hair should be assessed, because sometimes there is a hormonal cause behind it.

Types of acne and why it matters

Acne forms when a pore is blocked by dead skin cells and oil. What happens after that determines the type, and the type determines the treatment.

Comedones, meaning whiteheads and blackheads, are blockages without significant inflammation. Red and pus-filled spots mean bacteria and inflammation are involved. Cystic acne, meaning nodules and cysts, is much deeper inflammation and this is what most often leaves permanent scars.

That is why a doctor cannot give one prescription to everyone who comes in with acne. Treatment for comedones will not resolve cystic acne, and treatment for cystic acne is too strong for mild comedones.

What is usually treated at a general clinic

At a general clinic, acne treatment usually starts with topical medication that targets pore blockage or bacteria, and for more widespread or deeper cases, oral medication to reduce inflammation.

The doctor will also check other things that may be keeping the acne going: skincare products that block pores, medication you are taking, and in some patients, signs of hormonal imbalance that need further investigation.

Some strong acne treatments, particularly oral isotretinoin, require blood monitoring and strict safety measures, and are usually managed by a dermatologist. If your case points that way, the doctor will say so plainly and give you a referral.

Things that usually make acne worse

These come up most often in conversations at the clinic.

  • Changing products every two or three weeks before the previous one has had time to work
  • Squeezing spots, particularly the deep painful ones
  • Using scrubs or alcohol-based toners on already inflamed skin
  • Whitening creams with unclear ingredients, which often cause both acne and dark patches
  • Stopping medication as soon as the skin looks better, then the acne returns
  • Heavy powder and foundation to cover active acne

How long before you see results

Acne does not respond quickly. Most treatments take six to twelve weeks to show a change worth judging, and in the first two or three weeks some people look worse before they improve.

That is why the doctor will give a timeframe when starting treatment, and ask you to come back to review rather than changing medication each time.

We do not guarantee results. Anyone promising clear skin within a set time is not being honest about how acne behaves.

Common questions

My acne is severe. Do I need a skin specialist?

Most acne can be started at a general clinic. If your acne is the widespread deep nodular type, or has already failed several treatments, the doctor will tell you and give a referral letter to a dermatologist.

Does food cause acne?

The evidence for specific foods is weaker than people believe. Some studies link high sugar diets and dairy with acne in a proportion of people, but it is not the main cause for most patients. Treating acne through diet alone is rarely enough.

Can I use acne cream during pregnancy?

Some acne medications cannot be used during pregnancy or when planning a pregnancy. Tell the doctor if you are pregnant, breastfeeding or planning a pregnancy before medication is given.

From what age can acne treatment start?

Acne often starts in early adolescence and can be treated at that age. Medication choices differ for children and teenagers, so do not use adult medication without an examination.

If I stop the medication, will the acne return?

For some people, yes. Acne often needs maintenance treatment once it is under control. The doctor will explain the plan after the initial phase.

My acne left dark marks, not pits. Is that the same?

Not the same. Dark marks after acne are a colour change and usually fade with time. Pits are a change in the structure of the skin and are more difficult. Each has a different approach.

Do you do laser or peels at this clinic?

No. This clinic focuses on medical treatment for acne. Aesthetic procedures require separate approval under the aesthetic medical practice guidelines.

How many times do I need to come back?

Usually one follow-up after four to eight weeks to review, then as needed. The doctor will tell you when to return at the first visit.
